Southern Bone & Joint Specialists – Terms & Conditions

These Terms and Conditions govern the text message (SMS) program operated by Southern Bone and Joint Specialists, P.A. (“Southern Bone & Joint,” “we,” “us,” or “our”). By opting in, you agree to these Terms.
1. Program Description
The Southern Bone & Joint Specialists Review Request program sends a brief text message asking you to leave a public review of our practice on a review site such as Google, Yelp, WebMD, or Healthgrades. This is a one-way informational program. We do not send advertising or promotional offers, appointment reminders, billing notices, or clinical information through this program. The message text is generic and identical for every recipient: it contains no personal or health information and does not reference a visit, an appointment, a provider, or your relationship with the practice.
2. How to Opt In
You may enroll only by giving your affirmative, express consent in one of the following ways:
– Checking the text message consent box and providing your mobile number on a patient intake,
registration, or update form completed at one of our offices; or
– Checking the text message consent box in our online appointment scheduling system or patient portal.
The consent box is never pre-checked. You may complete any form, schedule any appointment, and
receive any service from us without checking it.
Consent to receive text messages is not a condition of purchasing any goods or services. No purchase is necessary. Consent is also not a condition of receiving medical treatment or of scheduling an appointment, and declining or withdrawing consent will not affect your care in any way.
By opting in, you represent that you are at least 18 years old and that you are the account holder or an authorized user of the mobile number you provided. If your mobile number changes or is reassigned, please notify us promptly at pr@southernbone.com so we can update or remove it.
3. Message Frequency
You will receive approximately 1 message per month. Message frequency may vary.
4. Cost
Message and data rates may apply. Southern Bone & Joint does not charge a fee for participating in this program. Any charges for sending or receiving text messages are billed by your mobile carrier under your wireless plan. Please contact your carrier if you have questions about your plan or charges.
5. How to Opt Out
You can cancel at any time. Reply STOP to any message from us. You may also reply STOP.
After you send one of these keywords, we will send a singleconfirmation message and you will receive no further messages from this program.
You may also opt out by calling [MAIN PHONE], emailing [SUPPORT EMAIL], or telling any member of our staff at any office.
If you wish to rejoin later, reply START or opt in again through one of the methods in Section 2.

7. Supported Carriers and Delivery
The program is supported on major U.S. mobile carriers, including AT&T, Verizon Wireless, T-Mobile, UScellular, and others. Carrier support may change without notice. Carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission by your carrier and is not guaranteed. We are not responsible for messages that are delayed, misdirected, or not delivered.
8. Privacy
Information you provide in connection with this program is handled as described in our
[Privacy Policy](https://southernbone.com/privacy-policy). No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. We share your mobile number only with the messaging vendors and carriers necessary to deliver the messages you requested.
Standard text messaging is not encrypted. Anyone with access to your mobile device or carrier account may be able to view messages we send you. Our messages are written to exclude protected health information for this reason.
9. Reviews
Participation in this program is voluntary, and you are under no obligation to leave a review or to leave a review of any particular kind. Reviews you post are governed by the terms and privacy
policies of the third-party review site, not by these Terms. We do not offer any payment, discount, or other incentive in exchange for a review, and we do not condition any aspect of your care on whether or what you write.
10. Your Device and Content
You are responsible for maintaining a device capable of receiving text messages and for any carrier charges you incur. Messages sent through this program are our property; you may not resend, alter, or commercially reuse them.
11. Disclaimer and Limitation of Liability
The program is provided “as is” and “as available.” To the fullest extent permitted by law, we
disclaim all warranties, express or implied, regarding the program, including warranties of
merchantability and fitness for a particular purpose. To the fullest extent permitted by law, we will not be liable for any indirect, incidental, consequential, special, or punitive damages arising from your participation in the program, including damages arising from delayed or undelivered messages. Nothing in these Terms limits any right you may have that cannot be limited under applicable law.
12. Changes to These Terms
We may modify or discontinue the program or these Terms at any time. Changes take effect when posted at this page with a revised effective date. Your continued participation after a change constitutes acceptance of the revised Terms.
13. Governing Law
These Terms are governed by the laws of the State of Alabama, without regard to its conflict of laws principles.
