About Your Visit

We look forward to your first visit and will help you in any way we can. To help us, we ask that if you have been seen by another doctor for the same problem that you bring a copy of your medical records and x-rays with you to your appointment.

Checking in For Your Appointment

Our online check-in process saves time by allowing you to upload your driver’s license, insurance cards, medication list, update past medical history and sign any necessary forms securely from any web-enabled smartphone or computer. Don’t have a smartphone or computer? No problem! You can use one of our devices.

You will receive a text/email message 4 days prior to your appointment. The message contains a link to complete your check-in paperwork prior to arriving to the office. If you do not complete the link prior to your appointment, please arrive 20 minutes early to access the link in our office.

Office Safety Policies

For your safety and the safety of our staff, we ask that you arrange for a caregiver to help you get in and out of your vehicle when you visit our office. We have wheelchairs available at the front entrance.

We also ask that all patients coming from Skilled Nursing/Rehabilitation facilities for appointments are accompanied by a caregiver upon arrival to our office and remain with the patient for the duration of the appointment. This is to assist with paperwork, communication with our staff, or assist with potential person needs during the patient’s time at our office.

Our facility is a healing environment. We understand injury can be stressful for all involved, however, aggressive behavior will not be tolerated. Click here to view our Disruptive Behavior Policy.

Prior Authorizations

Prior authorization in health care is a requirement that a provider (physician, hospital, etc.) obtains approval from your health insurance plan before prescribing a specific medication for you or performing a particular test or medical procedure. Without this prior approval, your health insurance plan may not pay for your treatment, leaving you responsible for the full bill. This process can take several days.

Prescription Refills and Contacting Your Nurse

We’ve made requesting prescription refills easy for our patients. You can request through the Patient Portal or call our office at (334)793-2663 and choose the option for Prescriptions. When leaving your message, please provide the following information to prevent delays in processing your request: patient’s name and date of birth, patient’s phone number, and a brief description including the name of the medication. Please allow 24-48 hours Monday through Friday for us to process your request.

If you wish to contact your nurse, you can do so through sending a message through the Patient Portal or you may call our office (334)793-2663 and choose the option for Nurse Message. When leaving a message, please provide the following information to help us better assist you: patient’s name and date of birth, patient’s phone number, and brief description of the reason for your call. Please allow 24 hours Monday through Friday for a nurse to respond to your message.
