Southern Bone & Joint is proud to offer online medical records requests. There are three types of records request services. See FAQs.
If you are our patient and would like to request your medical records, please click here to complete your request for medical records. You will be required to provide a valid email address and a government-issued ID such as a driver’s license or military ID.
The form goes directly to Sharecare for processing. Sharecare begins processing your request within 48 hours of submitting it. Records are usually available within 5 – 7 business days from the time the electronic request and payment is received.
Only the patient, parent/legal guardian, or the patient’s legal health care representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for healthcare/Conservatorship.

Once you have submitted your form, Sharecare will contact you within 48 business hours to collect payment for processing.

If you are not a Southern Bone & Joint Specialists patient and need to submit a request for the medical records of a Southern Bone & Joint patient, you may submit your request along with the patient’s authorization through our portal. Click here to submit a request for medical records.
After you have submitted the form with the patient’s authorization, Sharecare will contact you to move forward if payment is required prior to processing. Otherwise, Sharecare will promptly process your request.
