ENDOSCOPIC PROXIMAL HAMSTRING REPAIR REHABILITATION PROTOCOL

Precautions:
  • X-Act ROM Brace for 4-6 weeks (per physician recommendation)
  • WBAT with crutches for 2-4 weeks (per physician
  • Avoid active hamstring strengthening/hamstring stretching
Phase I – Acute Phase (weeks 0-6)

Goals:

  • Gradually improve weight bearing and ROM
  • Diminish pain and inflammation
  • Retard muscle atrophy and strength loss
  • Enhance healing of repair
  • Cryotherapy
  • e-stim for pain/swelling
  • Teach pt. to transfer safely
  • Gait training; restore normal gait

Exercises:

  • Gentle hip flexor, quad, calf, hip adductor stretches
  • Quad sets
  • Ankle strengthening
  • Isometric hip abd/add
  • Initiate core stabilization exercises
  • ROM: Gradually increase hip flexion (with knee flexed)
Phase II – Subacute Phase (Weeks 6-12)

Goals:

  • Improve flexibility and ROM
  • Enhance muscular strength and endurance
  • Promote healing
  • Control any inflammation and pain
  • Modalities as needed
  • Heat, ultrasound, ice
  • Soft tissue mobilization

Exercises:

  • Initiate isometric hamstring contractions (knee flexed to 45 degrees)
  • Progress to isotonic strengthening
  • SLR’s
  • Knee extensions
  • Backward lunges
  • Lateral step overs
  • Forward and backward step overs
  • Calf raises
  • Bridges
  • Wall squats
  • Lateral lunges
  • Leg press
  • Initiate proprioception drills
  • Iiniitate pool exercises, pool running or alter g running(with physician approval)
Phase III – Dynamic Phase (Week 12-16)

Goals:

  • Improve dynamic flexibility of hamstrings
  • Normalize static flexibility
  • Normalize muscular strength

Exercises:

  • Active warm-up
  • Stretch all major LE muscle groups
  • Continue strengthening exercises listed above
  • Jogging on dry land
  • Initiate light ballistic maneuvers when appropriate
  • Plyometric leg press
  • Plyometric front lunges
  • Scissor jumps
  • Scissor box jumps
  • Side to side box jumps
  • Skip lunges
  • Fast speed lateral step overs
  • Forward knee high running
  • Stretch after workout

Phase IV – Sports specific and agility phase (Weeks 16 and beyond)

  • Normalize ballistic flexibility
  • Normalize muscular strength and agility
  • Gradual return to sports participation

  • Continue selected ballistic drills

Pain Management Protocol

  • Narcotic pain medications will NOT be prescribed pre-operatively.
  • Even with the use of pain medication, it is not expected that you will be 100% pain free. Some discomfort is expected after surgery.
  • You will receive a narcotic pain medication for post-operative pain control, unless you request otherwise. This prescription is expected to last a minimum of 10-14 days (until your initial post-op appointment).
  • It is recommended that you take prescription pain medication, as prescribed, for the first 48-72 hours after surgery to “stay ahead of the pain.” If the pain gets out of control by not taking the pain medication, it will be more difficult to get it back under control. After 2 days, you can begin spacing out your dosages or taking less medication, as you feel comfortable.
  • Narcotic pain medication will not be refilled after 8 weeks post-surgery. If pain medication is still required at this point, you will be transitioned to a non-narcotic prescription.
  • We will discontinue the use of all pain-medications at 3 months post-op. If you believe you still require pain medication at this point, you may request a referral to a pain management specialist.
  • Medications will NOT be prescribed if you are not a current patient, being treated within the last 6 months. If you would like a long-term anti-inflammatory or any other type of medication, we suggest you discuss this with your primary care physician. We will manage antibiotic medication for joint replacement patients prior to having dental work.
  • Please keep in mind that some medications cannot be called in to your pharmacy, and may need to be picked-up in our office. Please allow time for processing.
  • If approved by your surgeon, you may supplement your prescription medication with an over-the-counter anti-inflammatory (Advil or Aleve – not Tylenol/acetaminophen) if a prescription anti-inflammatory is not prescribed. Alternate the dosages. Do not take antiinflammatory and pain medication in the same dose.
  • If a refill/change of medication is required, call the prescription line @ 334-793-2663 and allow 24 hours for your request to be processed.

Note: Many narcotic pain medicines can’t be called in to the pharmacy. We do not mail prescriptions. If you require a refill or a change of narcotic pain medication, you or a designee must pick up from our office. Picture ID is required.

Please keep your incisions clean, dry, & covered. Do not get it wet in the shower! No Neosporin, Vitamin E, or Scar Cream.

Apply ice 20 minutes of every hour (while awake) for the first 48 hours, then as needed for swelling. Do not put directly on the skin. No Heat!

You received Antibiotics through your veins while you were here, we do not routinely prescribe antibiotics post-operatively.

Most pain medicine contains Tylenol; please do not take Tylenol or acetaminophen with your pain medicine. It is ok to take Aleve or Advil. Please follow directions on the bottle.

Please call our office at (334) 793-2663 for prescription changes or refill requests. Please allow 24 hours for refill requests to be addressed. No pain medicine will be called in after hours or on the weekend!

Common Post-Op Prescriptions

You may receive some or all of these depending on the procedure you have done.Prescriptions that can be electronically prescribed will be sent to your regular pharmacy in advance.

Pain Medication: Percocet (Oxycodone) or Norco (Hydrocodone) – take per instructions as needed for pain

Anti-Nausea: Phenergan (promethazine) or Zofran (ondansetron)– take per instructions as needed for nausea

Muscle Relaxant: Robaxin (methocarbamol) or Flexeril (cyclobenzaprine) – take per instructions as needed for muscle spasm/tightness

Anti-Inflammatory: Mobic (Meloxicam) – HIP SURGERY PATIENTS – take as directed for 6-8 weeks post-operatively

ELBOW ARTHROSCOPY /ACCELERATED BICEP REPAIR

Post - Operative Care
  • You will be in a sling for 1 - 2 weeks following surgery . Wear at all times, except when bathing and doing physical therapy.
  • Ice the area several times per day for 20 minutes at a time (minimum of 20 minutes between sessions).
  • You should be able to return to work/school as soon as you feel comfortable, likely 2 - 5 days.
  • The dressings may be removed the day after surgery for bathing. If you are attending therapy the day after surgery, the therapist will remove the dressings for you, otherwise, you will be instructed how to manage this.
  • Do not get the incisions wet until your post - op visit. Cover with waterproof bandages when bathing.
Pain Control
  • Ice will help control swelling and pain .
  • We recommend you take the pain medication , as prescribed for the first 24 - 48 hours, then decrease as tolerated.
  • Schedule pain medication so that you can take a dose about 30 minutes prior to physical therapy sessions.
  • Even with pain medication, some discomfort or pain is normal . H owever , if you are experiencing a significant increase in pain, please notify our office at ( 334)793 - 2663.
  • You should receive a prescription for pain medicine and anti - nausea medication befor e leaving the hospital.
Infection Prevention
  • Infection prevention is always one of our primary goals.
  • You received an antibiotic through your IV at the time of surgery, therefore will not need an oral antibiotic
  • t is normal to have swelling, discomfort, some heat around the surgery site, a low grade fever (<100 degrees), and clear/pink drainage.
  • Notify our office if, after 24 hours, you have marked and sudden increase in swelling, pain, or heat in the joint or white/yellow discharge or persistent fever > 100 degrees after 325 mg of Tylenol.
  • Clean the incisions daily with betadine, allow to dry and cover with a clean, dry bandage.
Physical Therapy
  • You will begin physical therapy on the first or second business day after surgery. During the first session, the primary goal should be pain and swelling control and beginning to work through the prescribed rehabilitation protocol
  • Your first physical therapy appointment will need to be scheduled prior to surgery. We will assist you to determine your physical therapy location and will fax the necessary paperwork to the therapy facility. You will also receive a copy. Physical Therapy is typically recommended 2-3 times per week for 3-4 months.
Follow-Up
  • We recommend that you not drive until you are off all pain medication and can safely maneuver the car without putting yourself or others at risk.
  • Before leaving the hospital, you will receive your follow-up appointment information, surgery pictures (if applicable), discharge instructions, etc.

Post - Operative Care
  • You will be in a splint for 7-10 days after surgery. This should stay on at all times.
  • The splint will be removed at your first post-op visit and you will be transitioned to a post-op brace, cast or a new splint.
  • Ice the area several times per day,30 minutes at a time, over the dressings.
  • You should be able to return to work/school as soon as you feel comfortable, likely 2-5 days.
  • Upper body - You may wear a sling for comfort, but can discontinue as tolerated.
  • Lower body – Use crutches as instructed.
  • Keep the splint/dressings clean and dry at all times.
Pain Control
  • Ice will help control swelling and pain.
  • We recommend you take the pain medication, as prescribed, for the first 24-48 hours, then decrease as tolerated.
  • Even with pain medication, some discomfort or pain is normal. However, if you are experiencing a significant increase in pain after 24 hours, please notify our office at (334) 793-2663.
  • You should receive a prescription for pain medicine and anti-nausea medication before leaving the hospital.
Infection Prevention
  • Infection prevention is always one of our primary goals.
  • You received an antibiotic through your IV at the time of surgery and will not need an oral antibiotic.
  • It is normal to have swelling, discomfort, and a low-grade fever (<100 degrees)
  • Notify our office if you have marked and sudden increase in pain after 24 hours or persistent fever >100 degrees
  • Do not remove the splint or dressings. These will be removed at your post-operative visit.
Physical Therapy
  • Physical therapy will begin after your post-operative appointment.

  • We recommend that you not drive until you are off all pain medication and can safely maneuver the car without putting yourself or others at risk. At discharge, you will information about your follow-up appointment, surgery pictures (if applicable), discharge instructions, etc

Post - Operative Care
  • You will be on crutches for comfort, but able to put weight on the involved leg as tolerated.
  • Continue use of the crutches until you are seen for your post-operative appointment.
  • Wear your brace at all times for the first 2 days after surgery. You may remove it to shower and for therapy. After day 2, you may remove your brace to sleep as well.
  • Ice the area several times per day for 20 minutes at a time (minimum of 20 minutes between sessions).
  • You should be able to return to work/school as soon as you feel comfortable, likely 3-5 days.
  • Do not get the incisions wet until your post-op appointment. Cover with waterproof bandages for bathing.
  • It is normal to experience some groin/genital numbness after surgery. If experienced, this should resolve on its own within a few weeks of surgery.
Pain Control
  • Ice will help control swelling and pain.
  • We recommend you take the pain medication, as prescribed for the first 24-48 hours, then decrease as tolerated.
  • Schedule pain medication so that you can take a dose about 30 minutes prior to physical therapy sessions.
  • Even with pain medication, some discomfort or pain is normal. However, if you are experiencing a significant increase in pain, please notify our office at (334)793-2663.
  • You should receive a prescription for pain medicine, anti-nausea, muscle relaxer and an antiinflammatory medication before leaving the hospital. Please continue the anti-inflammatory until the prescription is gone, however you may discontinue the other medications as tolerated.

Post - Operative Care
  • You may put weight on the involved leg as tolerated.
  • Ice your hip if needed for 20 minutes at a time (minimum of 20 minutes between sessions).
  • You should be able to return to work/school as soon as you feel comfortable, likely 2-3 days.
Pain Control
  • Ice will help control swelling and pain at the hip aspiration site.
  • We recommend you take over-the-counter analgesics (i.e.: Tylenol) as needed for 24-48 hours, then decrease as tolerated.
  • Do not take prescription or over-the-counter anti-inflammatories for 6 weeks after your injection. This includes Advil, Aleve, Celebrex, Mobic, Naprosyn/Naproxen, etc.

What To Expect After Surgery

TOTAL HIP ARTHROPLASTY (HIP REPLACEMENT)

Day of Surgery
  • Most people go home the same day of surgery. Some people do require an overnight stay with discharge home the next morning.
  • You will begin oral pain medication immediately after surgery.
  • You will begin physical therapy and ambulate in the hospital prior to your discharge home. Therapy will be continued in an outpatient setting after discharge.
Post-Operative Care at Home
  • You will use a walker for 2-4 weeks, but will able to put weight on the involved leg as tolerated. • Ice the area several times per day for 20 minutes at a time (minimum of 20 minutes between sessions).
  • Do not get the incision wet until your post-op visit. You should be discharged from the hospital with a bandage or dressing. You may change the bulky outer dressing on post-op day 1. Under the outer dressing is a mesh dressing that will remain in place until directed.
  • You may shower on the second day after surgery. We ask that you please cover the incision with a waterproof bandage.
  • You will need to take a 325mg dose Aspirin daily for 6 weeks after your surgery. If you were previously on a different blood thinner, you will restart that on post op day #1 instead.
  • We recommend that you do not attempt to drive until you are: 1. Off pain medication, 2. If the involved leg is your right, you need to ensure that you can safely move your foot from pedal to pedal and your reflexes are well recovered – this may take up to 4 weeks. Check with your automobile insurance company before driving.
Pain Control
  • Ice, elevation, and compression will help control swelling and pain
  • We recommend you take the pain medication as prescribed for the first week after surgery, then decrease as tolerated. Do not skip any doses until your pain is well controlled.
  • Schedule pain medication so that you can take a dose about 30 minutes prior to physical therapy sessions.
  • Even with pain medication, some discomfort or pain is normal, however, if you are experiencing a significant increase in pain, please notify our office at (334)793-2663.
  • You should receive a prescription for pain medicine, Aspirin (unless on a previous blood thinner), and anti-nausea medication before leaving the hospital.

  • You received an antibiotic through your IV at the time of surgery, therefore you will not need an oral antibiotic.
  • It is normal to have swelling, discomfort, some heat around the surgery site, or a low grade fever (<100 degrees).

**You will need to take an oral antibiotic prior to any invasive dental or gastrointestinal procedures. Please notify our office where you would like the prescription called in, to be filled as needed, prior to these procedures. Please allow 24-48 hours for your prescription request to be processed.**

Day of Surgery – Immediate Post-op Instructions
  • Resume regular diet as tolerated.
  • Use ice pack every one to two hours, while awake, for 20 minutes.
  • Take pain medication as directed. It is helpful to take pain medication 30 minutes prior to physical therapy.
  • Resume normal medications as needed.
  • Use crutches as directed.
Post-operative day 1
  • Continue using ice pack as needed
  • Continue daily shower/band-aid routine.
  • Continue working with a physical therapist or doing home exercises as instructed.
  • Discontinue use of the crutches as tolerated.
Return Visit:
  • Before you are discharged from the surgical facility, you will receive an appointment to see us back approximately 2 weeks after surgery. If you do not receive this, please call our office @ 334-793- 2663 to schedule.
  • We recommend that you do not drive until you are off of pain medication for at least 24 hours and can do so safely.

Day of Surgery
  • Most people go home the same day of surgery. Some people do require an overnight stay with discharge home the next morning.
  • You will receive a long-acting numbing medication at the time of surgery that may last 18-24 hours. You will begin oral pain medication immediately after surgery.
  • You will begin physical therapy and ambulate in the hospital prior to your discharge home. Therapy will be continued in an outpatient setting after discharge.
Post-Operative Care at Home
  • You will use a walker for 2-4 weeks, but will able to put weight on the involved leg as tolerated
  • Ice the area several times per day for 20 minutes at a time (minimum of 20 minutes between sessions).
  • Use the ace wrap for compression until the swelling has resolved, which is usually 1-2 weeks.
  • Do not get the incisions wet until your post-op visit. You should be discharged from the hospital with bandage over the incision. You may remove the outer bandage on post-op day #2. Leave the mesh dressing that is underneath the outer dressing intact until instructed. Cover the incision with waterproof bandages for hygiene
  • You will need to take a 325mg dose aspirin daily until we instruct you to discontinue, which is usually 4-6 weeks. If you were on a blood thinner prior to surgery, you will resume that on the day after surgery instead.
  • We recommend that you do not attempt to drive until you are 1. Off pain medication, 2. If the involved leg is your right, you need to ensure that you can safely move your foot from pedal to pedal and your reflexes are well recovered – this may take up to 4 weeks. Check with your automobile insurance company before driving.

Hospital Stay
  • Expect to stay in the hospital for 1 night
  • You will receive a long-acting numbing medication at the time of surgery that should help control post-operative pain for 5 days or more. You will begin oral pain medication, as needed, immediately after surgery.
  • You will wear a shoulder sling at all times, except during physical therapy. You will begin physical therapy while you are in the hospital.
  • You may have drains placed at the time of surgery that will help prevent blood from pooling inside the joint. These will be removed the day after surgery.
  • If you require inpatient medical care (at a rehabilitation or skilled nursing facility) after discharge, the hospital will help you make these arrangements. Otherwise, outpatient physical therapy will be set-up prior to discharge.
Post-Operative Care at Home
  • You will wear the arm sling for 2-4 weeks. It should be worn at all times, including while sleeping, except for when bathing and doing physical therapy.
  • Ice the area several times per day for 20 minutes at a time (minimum of 20 minutes between sessions).
  • Do not get the incisions wet until your post-op visit. You should be discharged from the hospital with a nude colored bandage over the incision, and one additional bandage. Take the bandage off 5 days after surgery and clean the incision per the nurses’ instructions. Apply the new bandage and this will stay on until your post-op visit.
  • We recommend that you do not attempt to drive until you are:
  • Off pain medication for at least 24 hours,
  • You feel like you can safely do all the necessary tasks to drive safely.
  • This may take up to 4 weeks. Check with your insurance company before driving.
